Diabetes Educator (m/f/d)

Responsibilities
- Counseling and training of patients and relatives regarding diabetes mellitus
- Support with nutrition, medication, and lifestyle changes
- Interdisciplinary care in adult diabetology
- Creation of individual therapy plans in collaboration with doctors and departments
- Conducting training sessions and workshops on prevention and self-management
- Patient care including blood sugar monitoring and therapy adjustment
- Documentation and evaluation of consultations and patient data
- Collaboration in an interdisciplinary team
Profile
- Completed further training as a Diabetes Educator (DDG) or comparable qualification
- Experience in the care and counseling of diabetes patients
- Knowledge of modern diabetes therapy and insulin therapy
- Experience with CGM systems and digital applications
- Empathetic and communicative demeanor
- Enjoyment of working with patients and relatives
- Independent, structured, and reliable working method
- Teamwork skills and willingness for interdisciplinary cooperation
