Job description Family Support helps people in their daily life, with attention to care, well-being and household. As Coordinator Home Care family care, you are responsible for implementing the care and support of Family Support. You work together with colleague coordinators and care teams to provide the best care for clients in your region. In this temporary position, the focus is mainly on care coordination and care tuning. As coordinator home care family care in Family Support, you will be working closely with social workers, psychologists, and other professionals to ensure that our clients receive the right support at the right time. Your main tasks will include: • Coordinating the care process for clients, taking into account their individual needs and circumstances; • Ensuring that clients receive the necessary support and guidance from social workers, psychologists, and other professionals; • Maintaining a close relationship with clients, families, and caregivers to ensure that they are well-supported and informed throughout the care process; • Collaborating with colleagues and other stakeholders to develop and implement effective care plans; • Monitoring and evaluating the effectiveness of care plans and making adjustments as needed. Requirements: • Bachelor's degree in social work, psychology or a related field; • At least 2 years of experience in a similar role; • Strong communication and interpersonal skills; • Ability to work independently and as part of a team; • Good organizational and time management skills; • Knowledge of the GECO Statuut-Region Brussels area and its specific needs.
