About the role The Coordinator – Coasters Home & Community Support Services, is responsible for coordinating care to support consumers to live well, age well and have a respectful end of life in their own community. This includes working closely with consumers, families/whanau, and other stakeholders to develop and implement individualised care plans that meet the unique needs of each consumer. The Coordinator will also be responsible for ensuring that all care is delivered in accordance with the principles of Te Tiriti o Waitangi and the values of compassion, respect, and manaakitanga. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
