Community Worker

At New Dawn, we help people build independence and connection, guided by values of care, respect, courage and independence. We support NDIS participants through practical assistance, skills building and community inclusion.

What you’ll do

• Assess client needs and goals, then design individual support plans.
• Interview clients and families to understand strengths and challenges.
• Run community-based activities and workshops; review outcomes and adjust plans.
• Monitor progress and keep clear case notes and reports.
• Link clients to services for health, housing, employment and training.
• Work with community groups, providers and government services to improve access.
• Support families, children, young people and people with disability to participate.
• Help young people navigate social, emotional and financial issues.
• Prepare service reports and documentation to support funding and quality.

What you bring

• Cert IV or Diploma in Community Services, Social Work or similar, or equivalent experience.
• Experience in community, social or disability services.
• Great communication, cultural sensitivity and solid time management.
• WWCC, NDIS Worker Screening, First Aid/CPR, driver licence (or willingness to obtain).
