About the role
Maweja Super Care Pty Ltd is seeking a Community Worker to deliver structured
community access and social participation supports to NDIS participants and
community clients. This role focuses on helping individuals engage with their
community, access services, build independence, and achieve personal wellbeing
goals. The position is central to our disability and community services model and
aligns with professional welfare and community support practice.
About the service
Our Community Workers provide one on one and ongoing community based support
seven days a week. Supports may be delivered as single appointments or as part of
an ongoing program, depending on participant goals and needs. Services are
provided in community settings and clients’ homes and are designed to promote
inclusion, confidence, and independence.
Key Responsibilities
• Assess individual client needs, strengths, and barriers to participation through
direct engagement and ongoing review
• Plan, implement, and support community access and participation activities aligned
with individual goals
• Support clients to attend medical and allied health appointments and assist with
understanding and following up care requirements
• Assist clients with shopping, banking, and accessing essential community services
• Facilitate access to recreational, social, and cultural activities to promote social
connection and wellbeing
• Support clients to attend community facilities such as libraries, gyms, pools,
community groups, and events
• Liaise with families, carers, service providers, and community organisations to
coordinate supports
• Monitor client progress, wellbeing, and participation outcomes and report changes
or concerns appropriately
• Provide information, guidance, and referrals to relevant community and welfare
services
• Maintain accurate case notes, progress records, and reports in line with
organisational and funding requirements

• Promote client dignity, choice, independence, and safety at all times
Skills and Experience required
• Relevant qualification in community services, disability, welfare, or a related field, or
equivalent practical experience
• Demonstrated 2 years’ experience supporting people with disability or complex
needs in community settings
• Strong communication and interpersonal skills
• Ability to assess needs, plan supports, and work toward measurable participation
outcomes
• Capacity to work independently in the community and collaboratively within a team
• Sound organisational skills and accurate documentation ability
Requirements
• Current Australian driver licence
• Willingness to provide services in clients’ homes and community environments
• Availability to work rostered shifts, including weekends where required
• National Police Check
• Working with Children Check
• Blue Card
• Right to work in Australia
