Job description
At ACME Support Services, we help people with disability live with dignity, independence, and connection. We’re looking for a caring and capable Community Worker to join our team and make a real difference in people’s lives. What you’ll do Meet with clients to understand their needs, strengths, and goals. Create and deliver personalised support programs, including skill-building, social inclusion activities, and daily living assistance. Keep track of progress, write clear reports, and update periodically. Develop and maintain relationships with clients, families, and other stakeholders. Collaborate with colleagues to share knowledge, ideas, and best practices. Participate in team meetings and contribute to the development of new initiatives.
