Blossom Community Care is a registered NDIS provider offering a wide range of supports and services to help participants achieve their goals and live independently.
Role OverviewWe are seeking dedicated and compassionate Community Nurses to join our team.
In this role, you will deliver high-quality nursing care and clinical support to NDIS participants within their homes and the community. Responsibilities include assessing health needs, administering medication, developing and implementing care plans, providing wound care, monitoring health conditions, and supporting participants to manage their wellbeing in line with their individual NDIS goals.
- Passionate about delivering person-centred, high-quality nursing care.
- Genuine interest in improving the health and wellbeing of participants.
- Strong focus on participant outcomes and goal achievement.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Excellent communication and interpersonal skills.
- Strong clinical judgement, professionalism, and attention to detail.
- Proficiency in using digital tools, including clinical systems, smartphones, and computers.
- Ongoing professional development and clinical training opportunities.
- Supportive onboarding and induction to ensure a smooth transition.
- Flexible scheduling options to support work–life balance.
- Reimbursement for travel during participant visits.
- Easy access to rosters and documentation through our mobile app.
- Current AHPRA registration as a Registered Nurse (RN) or Enrolled Nurse (EN).
- Experience in community or disability nursing is highly regarded.
- Sound clinical knowledge and experience in areas such as medication management, wound care, continence management, and chronic disease support.
- NDIS Worker Screening Check.
- Current First Aid and CPR Certificate.
- Working with Children Check.
- Valid Australian Driver’s Licence.
- Reliable vehicle with comprehensive insurance and willingness to use for work.
- Reliable smartphone with data access.
- Demonstrates respect, empathy, and professionalism in all interactions.
- Provides clinical care aligned with individual support needs and NDIS guidelines.
- Works autonomously with a commitment to quality and safety standards.
- Strong documentation and record-keeping skills.
- Upholds confidentiality, ethical conduct, and compliance with organisational policies.
