This is a Clinical Nurse Consultant - RACH Liaison Nurse (531942) role with Department of Health, Tasmania based in Hobart, TAS, AU Department of Health, Tasmania Role Seniority - mid level, senior More about the Clinical Nurse Consultant - RACH Liaison Nurse (531942) role at Department of Health, Tasmania The Team The Residential Aged Care Home (RACH) Liaison team sits within the Integrated Operations Centre (IOC) at the Royal Hobart Hospital and supports safe, timely and well‑coordinated transitions for patients moving between residential aged care facilities and acute care settings. Key responsibilities include: • Providing expert clinical advice to RACH staff on patient care and treatment options • Facilitating communication and collaboration between RACH and acute care services to ensure seamless transitions • Developing and implementing strategies to improve patient outcomes and reduce hospital readmissions • Collaborating with other health professionals to develop and implement evidence-based practice guidelines for residential aged care facilities • Providing education and training to RACH staff on best practices in patient care and treatment options The ideal candidate will have: • A current registration as a Registered Nurse (RN) or Enrolled Nurse (EN) with the Nursing and Midwifery Board of Australia (NMBA) • A Bachelor's degree in nursing or a related field • At least 2 years' experience working in residential aged care facilities, acute care settings, or a combination of both • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment with multiple priorities • Ability to maintain confidentiality and handle sensitive information • Willingness to learn and adapt to changing circumstances The Department of Health, Tasmania offers: • A competitive salary package • Ongoing professional development opportunities • A supportive and collaborative work environment • Flexible working arrangements (e.g., telecommuting) • Access to a range of employee benefits, including health insurance, superannuation, and leave entitlements
