Tāmaki Health is seeking compassionate and clinically experienced Clinical Family Navigators to support patients and families through extended care in both home and clinic settings. This role is ideal for clinicians who are passionate about health equity, holistic care, and working closely with diverse communities.

About the Role

As a Clinical Family Navigator, you will support the clinical management of enrolled patients through an extended service model, primarily within the home environment and, at times, within clinic settings.

You will work closely with GPs, nurses, case managers, and community agencies to help patients and families navigate the health system, improve self-management, and achieve better health outcomes.

This role involves a mix of clinical support, patient education, care coordination, and community engagement.

What We’re Looking For

We are seeking motivated and patient-focused clinicians who:

  • Are passionate about supporting individuals and whānau in their home and community environments
  • Understand the social, cultural, and environmental factors impacting health
  • Are confident working independently while collaborating within a multidisciplinary team
  • Are committed to health equity and improving outcomes for Māori and Pacific communities
You Will Need to Have
  • A medical (MD) or registered nursing qualification (NZ or overseas)
  • Recent clinical practice in an acute care setting (within the last 2 years)
  • Minimum 2 years’ post-graduation clinical experience
  • Strong clinical knowledge and patient assessment skills
  • Ability to communicate effectively in at least one language other than English (preferred)
  • A current full New Zealand driver licence and ability to travel to patient homes
  • Legal right to work in New Zealand
  • Commitment to Te Tiriti o Waitangi and health equity principles
