Job description
Job Summary
You will play a key role in delivering high-quality patient care and supporting our clinical team at a busy GP practice.
Responsibilities- Lead nurse-led clinics focused on wound care, immunisations, and chronic disease support to improve patient health outcomes
- Assist clinical team by conducting health assessments and providing patient education to enhance patient understanding and self-care
- Maintain accurate and comprehensive clinical records in compliance with best practice protocols to ensure quality and continuity of care
- Collaborate effectively with healthcare professionals to support integrated patient management and clinical operations
- Registered Nurse with primary care or clinic experience
- Demonstrate a warm, professional, and patient-focused approach to foster positive patient interactions
- Exhibit strong teamwork and communication skills to contribute to a collaborative clinical environment
