Children’s Social Worker – Leaving Care – East Riding of Yorkshire

Job Title: Social Worker (Children) Location : East Riding of Yorkshire Working Hours: Hybrid Rate : £35 per hour An experienced Social Worker is required to join East Riding’s Pathways Leaving Care Team. This is an exciting opportunity to support young people in their transition to adulthood. Responsibilities: As a Social Worker within the Pathway Team, you will be responsible for working with young people who are looked after and care-experienced. You will provide assessments of needs for young people, develop and implement individual care plans, and work with other agencies to ensure the best possible outcomes for young people. You will also provide support and guidance to young people, helping them to develop the skills and confidence they need to make informed decisions about their lives. Requirements: The ideal candidate will have a degree in Social Work or a related field, and will have experience of working with looked after and care-experienced young people. You will also have a good understanding of the needs and experiences of young people in care, and be able to work effectively with them to achieve positive outcomes.
