Case Manager - Mental health Professional - Federal Government Department

- $55 - $65 an hour + Super
- Government Department role, long term contract
- Open to Psychologists, Social Workers, Mental Health Nurses, Occupational Therapists and Counsellors
Duties:
- Deliver high-quality, evidence-based clinical services, including risk assessment, brief interventions, care coordination, and referrals for clients with complex/multiple needs.
- Provide short-term, goal-oriented, clinical case management intervention to clients who present with unmet health and psychosocial needs that impact on their ability to access and/or meaningfully engage in mental health care.
- Maintain accurate and confidential client records, including progress notes, care plans, and risk assessments.
- Collaborate with internal teams and external organisations to ensure coordinated, client-centred support and effective referral pathways.
- Work within a stepped-care model, supporting vulnerable or at-risk communities and families.
- Participate in professional development and supervision to maintain registration and uphold best-practice standards.
- Contribute to service improvement initiatives, program delivery, and community engagement activities as required.
- Qualifications as a registered Psychologist / Mental Health Occupational Therapist / Mental Health Registered Nurse with full AHPRA registration, OR
- Qualification as a Social Worker with membership of the Australian Association Social Workers (AASW) on commencement OR
- A Masters-level counselling qualification and registration with PACFA as a Registered Clinical Counsellor or with ACA at Level 3 or Level 4
- Working with Children (WWC) and/or Vulnerable People Checks/Clearance must be obtained prior to commencement.
- As this role is within a Federal Government department it is a required to be an Australian Citizen at time of commencement
