Case Assessor

We are working with a local authority in the North West area seeking an experienced Case Assessor to join their Adult Social Care team. The role involves carrying out assessments and reviews to support individuals in maintaining their independence and accessing the right care and support services. Key Responsibilities: Undertake person-centred assessments and reviews under the Care Act (2014). Develop and monitor outcome-based care packages to meet individual needs. Work collaboratively with social workers, families, and other stakeholders to ensure that individuals receive the necessary support. Provide guidance and advice to colleagues on assessment and review processes. Identify and address any gaps in service provision. Maintain accurate records of assessments and reviews. Contribute to the development of policies and procedures for Adult Social Care. Key Requirements: Strong analytical and problem-solving skills. Ability to work independently and as part of a team. Excellent communication and interpersonal skills. Experience of working with vulnerable adults. Knowledge of the Care Act (2014) and relevant legislation.
