Care Manager RN - Oncology Clinic

Description Location: UCHealth UCHlth Memorial Hosp Central, US:CO:Colorado Springs Department: MHN Oncology Clinic Work Schedule: Full Time, 80.00 hours per pay period (2 weeks) Shift: Days Pay: $38.91 - $60.31 / hour. Pay is dependent on applicant's relevant experience This position is an onsite role and does not offer a hybrid or remote option Summary : Works collaboratively with physicians, interdisciplinary teams, individual clients and families to promote positive client outcomes. Performs comprehensive assessments of clients' physical, emotional, social and spiritual needs, develops and implements individualized care plans in collaboration with the healthcare team, and provides education and support to clients and their families. Provides direct patient care as needed, including but not limited to: administering medications, performing treatments, and providing wound care. Utilizes electronic medical records (EMRs) to document client information and communicate with other healthcare providers. Participates in quality improvement initiatives to improve patient outcomes and satisfaction. Maintains confidentiality of all client information and adheres to HIPAA guidelines. Collaborates with the interdisciplinary team to develop and implement discharge plans, ensuring a smooth transition for clients from hospital to home or other care settings. Provides education and support to clients and their families on disease management, symptom management, and self-care techniques. Participates in staff meetings and educational in-services to stay current on best practices and new developments in the field of oncology.
