The Role We are looking for an experienced Care Coordinator – Case Management to support the delivery of high-quality, person-centred care across our home and community services. Working with a multidisciplinary team, you will be responsible for the coordination of comprehensive and integrated care plans for clients, ensuring their needs and preferences are met. Key Responsibilities: * Conduct comprehensive assessments of client needs, goals, and preferences. * Develop, implement, and monitor individualized care plans in collaboration with clients, families, and healthcare professionals. * Coordinate and facilitate access to a range of services, including health, allied health, social support, and community resources. * Liaise with internal and external stakeholders to ensure seamless service delivery and effective communication. * Provide ongoing support, advocacy, and education to clients and their families. * Maintain accurate and up-to-date client records in accordance with organizational policies and procedures. * Participate in team meetings, case conferences, and professional development activities. * Identify and report any risks or issues to ensure client safety and well-being. * Contribute to the continuous improvement of service delivery through feedback and participation in quality initiatives. About You: * Demonstrated experience in case management, community services, or aged care. * Strong understanding of person-centred care principles and best practices. * Excellent assessment, planning, and organizational skills. * Proven ability to build and maintain positive relationships with clients, families, and stakeholders. * Effective communication, interpersonal, and negotiation skills. * Ability to work independently and as part of a multidisciplinary team. * Proficiency in using case management software and Microsoft Office Suite. * Current driver's license and willingness to travel within the region. * Relevant tertiary qualification in health, community services, social work, or a related field is highly desirable.
