As Begeleider D, you have a pivotal role in the life of adults and older adults with a mild to severe intellectual disability, combined with psychiatric problems such as autism and attachment problems. All clients have difficult-to-understand behavior that requires your expertise, calmness, and coordinating skills. Medical problems such as epilepsy or diabetes also occur.

In this challenging environment, you are not only a begeleider, but also a coordinator, connector, and director of care. You know what is needed to create safety, structure, and trust, and you put your professionalism into creating that foundation every day.

What will you do?

As Begeleider D, you are responsible for both the guidance and care coordination. You:

• Take on the role of personal guide and set up individual care and guidance plans that align with the specific needs of the client.

• Organizes and leads care plan discussions with colleagues and other disciplines.

• Serves as the point of contact for the client's network and other involved parties.

• Accompanies clients in daily activities, with attention to structure, safety, and clarity.

• Performs nursing skills on level 4 (if qualified and competent).

• Reports unusual behavior or health issues, takes action, and reports carefully.

• Takes the coordination of (part) projects within the team or domain.
