Job description
The social assistant's mission is to support individuals in addressing their social, administrative, and economic difficulties, by promoting their autonomy and inclusion. He/She supports access to rights, care, and social life, in conjunction with local partners. She collaborates with social, medico-social, and health professionals to propose comprehensive and personalized support. He/She brings his/her social expertise in compliance with the Association's values and the service's project. He/She engages in a process of good treatment and teamwork.
The social assistant assesses the needs and capacities of individuals in order to co-construct an adapted support project. She promotes autonomy, integration, and connection with partners, particularly in the field of care with the person's agreement. She ensures the follow-up, coordination, and evaluation of actions, while guaranteeing the traceability and quality of professional writings.
