Assistant social (H/F)

Your mission

• Evaluate the client's needs from a socio-economic point of view as well as their level of independence for the realization of administrative activities based on available resources.
• Restore conditions that allow clients, their entourage or a third person to manage administrative activities independently.
• Promote clients' rights vis-à-vis insurance companies and social aid organizations
• Contribute actively to maintaining clients at home through specific measures while taking into account individual needs with personalized care and promoting autonomy.
• Facilitate procedures for day care or temporary or long-term accommodation.
• Stimulate and support clients and their loved ones by maintaining maximum autonomy and quality of life according to their capacities.
• Collaborate actively with interdisciplinary teams, playing a specific role that allows you to value your social worker's job as an integral part of the healthcare system.

Your profile and skills

• Bachelor in social work with a focus on social services (or SECO recognition)
• Proven experience of 2 years in the medical-social field
• Marked interest in community health
• Able to collaborate in network and pluridisciplinarity
• Sense of responsibility, ability to organize autonomously and set priorities
