What You Will Do: Provide person-centered quality support services to individuals with a disability, including personal care, domestic assistance, meal preparation, and social support. Support clients to set and reach goals that are important to them, and make decisions about how best to live the life they choose. Develop and maintain effective relationships with clients, families, and other stakeholders. Participate in regular team meetings and contribute to the development of individualized care plans. Assist with daily living tasks such as personal care, meal preparation, and household management. Provide emotional support and companionship to individuals with a disability.
