The Saphir Foundation, a multi-mission and multi-site foundation (psychogeriatrics, geriatrics, adult psychiatry), is active in the medico-social field in the canton of Vaud and in the reception and support of vulnerable individuals in their physical and psychological integrity throughout their life process.

We are looking for for our establishment, EMS Montchoisi in Orbe, which accommodates 84 people and specializes in the field of advanced age psychiatry and geriatrics:

Community Health and Care Assistant (M/F/D) in Permanent Contract between 80% and 90% - EMS Montchoisi

Regardless of their role, the professionals of the Saphir Foundation share a common desire: to support individuals on a daily basis to enable them to concretely experience the institutional strategy of services "like at home". As a Community Health and Care Assistant, you will be responsible for supervising and supporting individuals in their daily lives and will participate in their overall care by assessing their physical, psychological, social, and spiritual needs. You will participate in the development of care plans and will administer delegated medical acts under the delegation of the registered nurse, and you will know how to prioritize interprofessional collaboration. Furthermore, you will participate in the supervision of new professionals, apprentices, and trainees in the care service.

YOUR BACKGROUND - YOUR QUALITIES AND SKILLS

• Federal Diploma of Community Health and Care Assistant
• You have an interest in the medico-social field, particularly geriatrics
• Your attitude is centered on the well-being of the individual
• You are flexible, adaptable, and organized
• Communication, teamwork, and the ability to self-reflect are qualities that define you
• You master usual computer tools (Carefolio) and have a perfect command of the French language
