Description of the post In the framework of collaborations with its partners in the medical-social sector, Medicalis Neuchâtel is actively seeking a Community Health and Social Services Assistant (ASSS) to intervene within an institution specializing in accompanying people with disabilities, in the canton of Neuchâtel. Accompany people with disabilities in their daily life tasks Ensure basic care and follow-up on health status Collaborate with healthcare professionals and other stakeholders to develop personalized care plans Provide information and support to individuals and families Develop and maintain relationships with community resources and services Participate in multidisciplinary teams to develop and implement care plans Monitor and evaluate the effectiveness of care plans Identify and report any changes or concerns related to the individual's health status
